The Leadership at Duncan Regional Hospital cares about the health, quality and continuity of care provided to our team members. The Care Coordination program has been developed to assist  team members with improving their health by empowering them to actively partner with their healthcare team to achieve their wellness goals.

What is the Cost?
Free for insured team members and/or their covered dependents.

Qualifications are:

Anyone enrolled in the Duncan Regional Health Plan and who is diagnosed with one or more of the following chronic health conditions:

Diabetes, pre-diabetes
Cardiovascular conditions
Benefits of Participation
Members who qualify for the program are contacted by direct mail, telephone and e-mail for the purpose of providing education, health coaching and self-management support to assist them in achieving healthy outcomes. Other benefits of the program include the following:

Financial incentives for participating
Collaboration with your provider to strengthen the patient provider relationship
Case management services to assist in the continuum of care

The Care Coordination Program includes certain financial incentives for participation. The services below may be eligible for reimbursement:

Provider office visits
Diabetic Supplies
Diabetic Education
Diabetic Eye Exams
Other Free Incentives:

Full Plate Education
Dietary Recommendations
Exercise Recommendations
Wellness Calls/Visits with the Wellness Navigator

Complete the Care Coordination Enrollment Form found here
Complete and return a release of information form
Requirements of Participation

Actively participate by accepting and returning Wellness Navigator calls
Complete the Wellness Program and attend scheduled provider visits
Utilize a Duncan Health Partners Provider
